Pay Scheduling Fee

 Scheduling fee is NON-REFUNDABLE

– Scheduling fee is required for all new clients to schedule an office visit or surgery. Please do not pay until you have scheduled an appointment, and have been instructed to pay per staff member.

–  If you need to reschedule or cancel, please notify us 36 hours in advance of the scheduled appointment time and the prepayment will remain as a credit on the client’s account

–  If you cancel without sufficient notice or simply fail to show up for the appointment, the advance payment is forfeited and there will be no credit to the client’s account. 

– We reserve the right to require this scheduling fee for existing clients as well.
